骨转+脑转+胸腔积液+肺部炎症，下一步怎么办，求助！
患者现年69岁，18年8月查出肺癌晚期骶椎和纵膈转移。基因检测19缺失。1、服易瑞沙4个月，开始多发骨转移。
2、19年1月下旬盲吃特罗凯184（40mg），肋骨疼消失，但咳血；
3、2月中旬开始盲吃：9291（标准剂量）+280（200mg*2次）。
4、4月1号开始9291单药120mg。
2019年2月第二次基因检测，发现19突变（丰度1.1%）和pten（丰度2.42%）突变。
从患病到现在双腿一直麻木轻微疼痛，服用280后有缓解但20天后腿部又加重，并且肋骨再次痛。检查出现胸腔积液，肺部炎症。近几天腿部疼痛厉害，干咳厉害，脸部浮肿，尿液少，食欲下降，走路不稳，没精神，白天嗜睡状。

请各位大神支招 肾功能应该是受到影响，用点利尿剂呋塞米和螺内酯40：100服用，或者直接输入地米！考虑肺部炎症加重，且280控制效果一般，可考虑加入1120联合，形成e+met+v，1120好像可以改善肺部功能的作用，不过注意副作用，仅供参考！ 谢谢 有类似的pten突变吗？
